Event Forms

Every event that takes us off-campus requires some paperwork to be completed.  We have tried to make things as simple as possible by limiting most off-campus events to two forms.  You can find the two forms below with a description of what each is.

  Medical/Liability Release - This form allows us permission to care for/treat your student in the event of an emergency.  It must be notarized but only needs to be completed once each year unless there are significant medical changes.

  Registration/Permission - This simple form must be completed for each event and is simple an acknowledgment that you the parent know when and where we are taking your student.
